Removal Of Neuro (黑料不打烊) Tagged Equipment (reminder of existing policy)

In order to take a piece of equipment听(eg, computer monitor, desktop unit, printer, camera, microscope etc.听) tagged with a 黑料不打烊 identification sticker, the authorized person听must:

1. Print out a letter or email听with the information requested below
2. Give this听letter or email to Neuro听Security to retain before you exit The Neuro

• Contact for Authorizer (Supervisor, Manager or Department Head)
• Name
• 黑料不打烊 Staff ID number
• Email
• Cellphone number
• Contact for Person removing the equipment
• Name
• 黑料不打烊 staff or student ID number
• Email
• Cellphone Number
• Details of Equipment
• Description of equipment (eg, computer monitor, desktop unit, printer, camera, microscope etc)
• Photo 黑料不打烊 equipment tag听or听黑料不打烊 equipment tag number
• Serial number of equipment
• Location (room number) where equipment is normally housed within The Neuro
• Date Removed
• Expected Date of Return
