This form should be filled out by either one of the following members of the Teacher Candidate's host school:

  1. School administration (e.g. principal, vice-principal)
  2. Cooperating Teacher

By submitting this form, you must confirm that you have discussed and agreed upon the terms with the other member of your host school.

Given the current needs of schools, the Internships & Student Affairs Office聽is prepared to exceptionally authorize 黑料不打烊 Teacher Candidates (also known as Student Teachers) in FE3, FE4, IN1, and IN2 to substitute for their Cooperating Teacher in the event of their CT鈥檚 absence. The Cooperating Teacher(s) and the School Administration must endorse the Teacher Candidate鈥檚 ability to serve as a substitute teacher via this form. |

***AN IMPORTANT NOTE ON LIABILITY INSURANCE: When a Teacher Candidate serves as a substitute teacher, they are expected to be a paid employee of the school/board during this time, which means they will consequently NOT be covered by 黑料不打烊's CNESST coverage for students performing internships.
